PATIENT PORTAL
Once you make an appointment with us, we will construct a patient portal for you to sign in with. The username will be as follows: firstnamelastname2348@wellnesszone101.com. The password will be as follows: firstletteroffirstnamelastname2348. Once we have set this up for you, you will be able to sign in and fill out your health intake form prior to you coming in to see Dr. Lindsey. This will hasten your appointment as she can look over it prior to you coming in. If you don't like working on computers, you are able to fill it out on paper here at the office. We just ask that you arrive 20 minutes early to have enough time to do this. We look forward to helping you build your own chiropractic story!
